GLL TERMS AND CONDITIONS OF MEMBERSHIP ? JANUARY 2012

This document specifies the Terms and Conditions of all memberships. The Member is advised to read the whole of this agreement carefully. This agreement is made between GLL and you (The Member).

1. You, Your Health and Your Safety

1.1 The Member chooses whether GLL can assess his/her health and fitness level and prepare an exercise programme designed with the intention to improve his/her fitness level. The Member accepts and fully understands the following:

1.1.2 That if the member decides to Opt out of the induction, a disclaimer must be agreed.
1.1.3 That as the member has agreed the disclaimer; GLL cannot accept responsibility for any injury, loss or damage sustained as a consequence of misuse of equipment or as a consequence of an existing health condition.
1.1.4 That the Member enters an exercise programme and uses any equipment entirely at his/her own risk.
1.1.5 That any exercise programme is designed to place an increasing workload on the body's heart and lungs. The exercise programme cannot be predicted with complete accuracy and there is risk that the applicant may experience abnormalities in blood pressure and heart rate before, during or after exercise.
1.1.6 That the members who choose to have a fitness induction, a specific exercise programme will be given in response to the information provided on the Health and Fitness Questionnaire.
1.1.7 That all exercise programmes will include a warm up, a specifically designed exercise programme and a cool down.

1.2 The member hereby authorises GLL to obtain any additional information which it may require from his/her General Practitioner (GP) or ask the member to produce this.

1.3 A GLL fitness instructor must be consulted before using any new piece of fitness equipment. GLL cannot accept responsibility for any injury sustained as a consequence of any item of equipment being used upon which the member has not received instruction.

2. Your Health

2.1 The member will be responsible for monitoring his/her own physical condition throughout any exercise programme. In the event of any unusual symptoms occurring, the member should inform a GLL member of staff immediately or raise the alarm.

2.2 The member who chooses to have an induction declares that to the best of his/her knowledge, the information provided in the health & fitness questionnaire is complete and accurate. The member must disclose to GLL if he/she knows of any reason why he/she should not take part in the exercise programme prescribed.

2.3 If the member chooses to deviate from any prescribed exercise programme in any way he/she does so at their own risk.

3. Our Centre Conditions

3.1 Members are required to adhere to the centre rules and regulations on display at each centre. Failure to do so may result in entry being refused. The refusal of entry duration is at the discretion of GLL.

3.2 Admission will not be granted unless a valid membership card is presented clearly to the receptionist or validated by the computer system at the kiosk each time you use the facility.

3.3 Membership cards will not be valid until the member has their photograph taken.

3.4 In the event of losing your membership card, please visit your local leisure centre. A replacement card will be issued and a small administration charge will be made.

3.5 Your membership card is non transferable. You must not allow anyone else to use your card or membership number. Cards being used by any other person other than the authorised member will result in that membership being cancelled and no refund of the registration fee or subscriptions made.

3.6 Admission is subject to the facility operating at a safe capacity. The decision of the Centre Manger or their nominated person is final.

3.7 Centre programming is subject to change and availability from time to time.

3.8 Membership products and prices may be subject to change at the discretion of GLL.

3.9 All members must wear wrist bands on each visit to the Fitness Centre. These are collected on arrival and must be worn on the wrist. Refusal to wear a wrist band will result in your membership being revoked.

3.10 Members are required to change into training clothing. Men must wear tops or vests.

3.11 Members are asked to respect personal hygiene at all times within the centre and to wipe clean equipment after use.

3.12 Free weights are to remain on the matted area at all times and be returned to the racks after use. Additional weights being loaded onto the resistance machines are prohibited.

3.13 Lockers located near the fitness centre are for the use of the members using the fitness centre, members using other facilities are respectfully asked to use other centre lockers. You must not take locker keys off of the premises. All lockers will be emptied each night and items found placed in lost property. GLL cannot be held responsible for loss of or damage to any contents placed within the lockers, how so ever caused.

3.14 Non members, guests or friends must be referred to the customer service advisor on arrival. No spectators are allowed into activity areas.

4. Payment Conditions

4.1 Customers joining on a prepaid membership option must pay monthly by direct debit. Failure to make any due payment will result in the membership being suspended. Access will be denied until outstanding payment has been made. Any further outstanding account arrears will be payable before reinstatement of any membership scheme run by GLL. GLL retain the right to recover all outstanding balances.

4.2 Monthly direct debits are a full binding contract between GLL and (the Member) and will automatically continue until GLL are notified otherwise in writing.

4.3 All centre and membership prices are reviewed from time to time. If your membership subscription price is changed GLL will notify you at least 10 days working days in advance by email and or Letter.

4.4 Members are required to pay for all activities outside of their membership option and must have a valid ticket for use.

4.5 Membership joining fees are non refundable. Annual Prepaid memberships are not refundable.

4.6 In the event of a membership subscription expiring or due payments not being made new joining fees will be charged, where applicable.

4.7 All partner membership (except the premier Partner Upgrade), can only be processed through one bank account and therefore one direct debit mandate.

5. Change of Membership Status

5.1 All enquires relating to a membership purchased in your leisure centre please contact Customer & Online Services team via cust...@gll.org

5.2 All prepaid membership cancellations must be made in person at your local leisure centre. Alternatively you can request to cancel your membership via email . Dependant on your chosen cancellation date you may be required to give up 6 weeks notice to terminate your Direct Debit and membership. You will receive confirmation via return email.

5.3 Members are also required to cancel their direct debit instructions (DDI) with their bank to fully complete the cancellation process.

5.4 Direct Debit memberships are non refundable if the full cancellation process is not completed.

5.6 Membership suspension will be considered if the member cannot continue participating for medical reasons. This requires a letter from your Doctor and will only apply from the date of receipt of the letter. A letter from your Doctor will also be required on your return to fitness. If you require your membership to be frozen for non medical reasons you must apply in writing to Customer & Online Services team via cust...@gll.org. Memberships can be frozen for a maximum of 3 months within a 1 year period commencing from the membership start date (Eligibility for freeze is restricted and supporting documentation will be required. Please see your leisure centre for details). A ?freeze fee? will be charged each month via Direct Debit for non medical freeze reasons.

5.7 Should GLL choose to discontinue any of our membership schemes the member will be notified in writing one month in advance.

5.9 Should GLL be subject to a contract transfer into operating the borough leisure centres, your Terms and Conditions will be automatically updated to GLL?s Terms and Conditions as stated.

6. Booking Conditions

6.1 Crèche booking restrictions (parent/guardian, max of 2 children per person/membership). Care nannies may bring the child they care for on their prepaid card, but only after written approval from the general Manager. There is a maximum of 2 hours per session per day (premier prepaid customers). Maximum if 2 hours per week (wellness prepaid customers).

6.2 Court booking restrictions allow only 1 court per activity per day per member. These can only be booked form 6 days in advance and the member booking the court must play.

6.3 Members may make single activity bookings 6 days in advance. Multiple bookings can be made from 5 days in advance. Any payment due for additional bookings must be made at the time of admission.

6.4 Any activity not cancelled with 48 hours notice or not attended will be charged at the full rate for ALL members.

6.5 Online bookings are available to all members by registering online at www.better.org.uk . Online booking terms and conditions are also available.

The Management reserve the right to reject any application or withdraw without refund any membership in the event of failure to comply with Membership Conditions and Centre Regulations. GLL shall not be liable for any loss, damage of theft of personal property belonging to the member, (or any guest of the member) occurring upon the premises, except to the extent that such loss, damage or theft of personal property arises from any negligent act or omission of GLL. GLL Leisure Centres shall not be responsible for the death or injury of the member, (or any guest of the member) occurring upon the premises or as a result of the use of the facilities and/or equipment provided in the premises, except to the extent that such death or personal injury arises from any negligent act or omission of GLL Leisure Centres. For the purpose of this clause GLL includes Leisure Centres or any premises operated by it, Health and Fitness specialist instructors or other employees, servants or agents (including any independent contractors from time to time employed by it) and Premises means any premises operated by the company.
